Dear Parent/Guardian:

EVERY Sport Season (Fall, Winter and Spring) you will need to register through Genesis for all athletic programs. This must also be done in addition to the annual Genesis School Registration.

At the beginning of the following months, you will receive an email from noreply@nvnet.org alerting you to register your child for that season:

Fall Sport Season – June 2024
Winter Sport Season – October 2024
Spring Sport Season – January 2025

Genesis sports registration is where you choose the sport your child will be participating in and how the team rosters are generated. You must do the Athletic Forms section of Genesis Registration to be cleared to participate.

The ATHLETIC registration must be completed by the following dates in order for your child to participate.

Fall Sport Season – 7/20/2024
Winter Sport Season – 10/15/2024
Spring Sport Season – 2/2/2025

All athletes must also have a valid physical on file with original signatures, dated after:

Fall Season: – 8/12/23 (Football/Tennis), 8/19/23 (All other sports)
Winter Season: Ice Hockey/Swimming/Bowling – 11/25/23;
All other sports – 12/2/23
Spring Season: 3/10/24 (All Sports)

This includes the Pre-participation PHYSICAL EXAMINATION FORM and HISTORY FORM which must be signed by the athlete and a parent/guardian. These documents are due to the School Nurse the same date that the Genesis Athletic Registration is due. Any submissions after that date will result in a delay of clearance to participate.

All physical forms may be picked up in the Health Office or Athletic Office, or accessed at DOWNLOAD HERE or at OTHS.NVNET.ORG, Health Services - Forms, or print them out from Genesis. The State physical forms must be used. We will NOT accept photocopies, scans or faxes. All signatures must be original. The Cardiac Assessment module must also be completed and signed by your physician.

The Preparticipation Physical Evaluation HISTORY FORM must be completed and reviewed by your physician when a student receives a physical and submitted along with the Preparticipation Physical Evaluation PHYSICAL EXAMINATION FORM.

According to State Law NO student will be allowed to participate in interscholastic sports (including tryouts and practice) until Northern Valley-Old Tappan’s school physician has signed the student’s physical exam form. The school physician will be available to sign the physical exam forms prior to the start of the each season. If your physical form is late, he will only sign these forms on Tuesdays during the school year.

A physical is good for one calendar year. Do not submit a new physical for each sport, unless your child’s current physical has expired or is about to expire. Both physical and history forms may be mailed to the school during the summer.  Please send them to the attention of the School Nurse.
